Central Coast Health Connect account

How to release and request medical records
Send your information to a provider or request copies of your medical records from 蒙太奇的健康 through the following process:

When will I get my medical records?
After you submit your request form, expect a Community 医院 staff member to contact you to let you know when your records will be ready.

Opting out prevents your personal health information from being shared, with two exceptions:*
1. The opt-out does not apply when the disclosure is made to public health authorities and the purpose of the disclosure is permitted by either HIPAA or applicable California law.
2. Your information may be disclosed to facilitate emergency medical treatment.
